Put these two side by side and they look almost the same: a red plastic body, a clear cap and a stainless steel ball inside. That is because they work the same way. AirPhysio is an oscillating positive expiratory pressure (OPEP) device sold for clearing mucus, with a real FDA clearance for that use. Better Breathing Sport uses the same steel-ball principle, adds a second ball for a different resistance, and is sold to athletes with promises about oxygen delivery, lactic acid and recovery.

So the question is not which mechanism is better. It is which job you need done, and which seller describes that job honestly. The scorecard above sums up the rounds. Which one is better for clearing mucus?

AirPhysio. When you breathe out through either device, the steel ball lifts and falls rapidly, sending vibrations back into the airways while creating gentle back-pressure that helps keep them open. Respiratory physiotherapists have used this technique for decades to help people with chronic mucus cough it up.

AirPhysio received FDA 510(k) clearance K203209 in January 2021 for "improving the clearance of mucus secretions from the airways", by showing it was substantially equivalent to the Flutter. Clearance is not approval of every claim on its sales page, and its own filing lists it as a prescription device, yet it is sold directly without one. Owners with chronic bronchitis or bronchiectasis often say they cough mucus up more easily, though studies of OPEP devices are small and mixed.

Better Breathing Sport can loosen mucus the same way, and our review notes it may help athletes who get mucus on the chest after colds or heavy training. It is simply not sold, labelled or cleared for that job.

Does either one improve athletic performance?

No published evidence shows that either one does, so this round is a tie at zero. Better Breathing Sport's page says it optimises oxygen delivery, reduces lactic acid and improves recovery and sleep. Lactate builds up during hard effort and your body clears it afterwards, faster with an easy cool-down; breathing through a device for a few seconds before exercise does not change that.

The research its marketing leans on is about a different tool. Meta-analyses of breathing-muscle training in athletes mostly studied inspiratory muscle trainers, which add resistance when you breathe in, used daily for four to six weeks with resistance increased in small steps. They show modest endurance gains, especially in less trained people. Better Breathing Sport adds resistance when you breathe out, for a few seconds before a workout. Its endorsement from former Australian football star Tim Cahill, who has also been linked to AirPhysio in news coverage, is paid marketing, not evidence.

How are they different in the hand?

Better Breathing Sport is more adjustable. Retail listings describe a kit with two steel balls: the larger one is heavier and needs more effort, and you swap to the medium one if you cannot keep a steady vibration. That is still only two fixed settings, not the gradual progression real training devices use.

AirPhysio has one ball, and you change the resistance by tilting the device as you breathe out. It is pocket-sized, has no batteries or refills, and comes apart for washing in warm soapy water. Clean either device after every use when you are unwell, let the parts dry fully, and never share one, because both come into contact with saliva and mucus.

Whose claims hold up better?

AirPhysio, though both stretch the truth. AirPhysio's FDA clearance is narrow, and its sales page goes far past it into asthma, colds, flu and "restoring lung capacity". But the product itself is backed by a regulatory filing and a large owner base: 7,181 Trustpilot reviews on a claimed profile that replies to complaints. One in five of those reviews is one-star, with complaints about cracking parts, delivery and price.

Better Breathing Sport's whole pitch is the performance claim, and that is the part with no evidence. We found no independent owner rating for the product, and one Australian retailer lists it as discontinued.

The safety rules are the same for both. Avoid them with a collapsed lung, if you are coughing up blood, after recent surgery to the face, sinuses, skull, ears or oesophagus, or with a ruptured eardrum or right-sided heart failure. Ask a doctor first if you have any lung or heart condition.

Which one should you buy?

Buy AirPhysio if you have a condition that produces a lot of mucus, such as chronic bronchitis or bronchiectasis, and your doctor or respiratory physiotherapist agrees.
